1. Eligibility: The Survey Sampling International Prize Draw (the 'Prize Draw') is only open to legal residents of the fifty (50) United States and the District of Columbia, Canada, the United Kingdom and the Netherlands. To participate, legal residents of the fifty (50) United States and Canada must be at least thirteen (13) years of age, legal residents of the Netherlands must be at least fourteen (14) years of age and legal residents of the United Kingdom must be at least sixteen (16) years of age. Employees of Survey Sampling International, LLC, ePrize, LLC and their parent, subsidiary and affiliate companies and suppliers, vendors and agents, as well as their family and household members are not eligible. Subject to all applicable laws and regulations. Void where prohibited. Participation constitutes the entrant’s full and unconditional agreement to these Official Rules and the Sponsor’s (as defined herein) and Administrator’s (as defined herein) decisions, which are final and binding in all matters related to the Prize Draw. Winning a prize is contingent upon fulfilling all requirements set forth herein.

3. Timing: The Prize Draw begins at 00:00 Greenwich Mean Time ('GMT'), 1 July 2011, and ends at 23:59 GMT, 30 June 2012 (the 'Promotion Period'). The Prize Draw consists of four (4) entry periods (each, an 'Entry Period') as outlined in the chart below:

The Sponsor's computer is the official time-keeping device for the Prize Draw.

4. How to Enter:

a. Through Survey Advertisements: During the Promotion Period, locate an online advertisement (e.g. a link or web banner) hosted on a website mentioning the Prize Draw. When you click on the advertisement you will proceed either: (1) to an intake questionnaire, which may request the disclosure of certain demographic and personal information (e.g. e-mail address), and then to a survey opportunity or (2) directly to a survey opportunity. Each eligible entrant may receive: one (1) entry into the Prize Draw for the applicable Entry Period for completing a survey or receiving a 'screened out' notice or message or receiving a 'quota full' message regarding a survey. You will not receive an entry for completing an intake questionnaire.

b. Through a Panel: During the Promotion Period, the Sponsor will operate country-specific Panels for which individuals have registered or may register. The Promotion Period consists of four (4) separate Entry Periods. The prizes for which you are eligible depend on your country of residence as set forth in Section 6 below. Limit: New Panellists registering with an applicable Panel during the Promotion Period may receive: one (1) entry into the applicable Entry Period draw for registering and completing the registration survey, if any. Each eligible entrant may receive: one (1) entry into the applicable Entry Period draw for completing a survey or receiving a 'screened out' notice or message or receiving a 'quota full' message regarding a survey and one (1) entry into the applicable Entry Period draw for updating his/her profile upon request of the Panel, if any. 

c. Through Mail-In Entry (only for U.S. residents): To enter without completing a survey, hand print your name, address, day and evening phone numbers, e-mail address (if any), and age on a 3" x 5" piece of paper and post it with the proper postage to '2011-2012 Survey Sampling International Prize Draw', PO BOX 5046, Department 575522, Kalamazoo, MI 49003-5046. You will receive one (1) entry into the applicable Entry Period draw. Limit: One (1) per envelope. All mail-in entries must be handwritten. All mail-in entries must be postmarked and received by the dates specified in the chart above. All entries become the exclusive property of the Sponsor and none will be acknowledged or returned. Proof of sending or submission will not be deemed to be proof of receipt by the Sponsor. The Sponsor is not responsible for lost, late, incomplete, invalid, unintelligible, illegible, misdirected or postage-due entries, which will be disqualified.

For All Methods of Entry: Each eligible entrant must have his/her own  e-mail address, separate from the  e-mail address of any other eligible entrant. Any attempt by any entrant to obtain more than the stated number of entries by using multiple/different  e-mail addresses, identities, registrations and logins, by joining multiple Panels or any other methods, will void that participant’s entries and may subject such individual to additional sanctions or penalties in accordance with the applicable Panel Terms and Conditions (if applicable). Use of any automated system to participate in the Prize Draw is strictly prohibited and will result in disqualification and all entries submitted or received via such automated system will be void. Neither the Sponsor nor the Administrator are responsible for lost, late, incomplete, invalid, unintelligible or misdirected information or entries, which will be disqualified and void. In the event of a dispute as to the identity of any entrant, the authorised account holder (as defined herein) of the e-mail address used by, or connected to, the eligible entrant will be deemed to be the entrant. The 'authorised account holder' is the natural person assigned an  e-mail address by an Internet access provider, online service provider, Internet service provider or other organisation responsible for assigning  e-mail addresses for the domain associated with the submitted address. Potential winners may be required to show proof of being the authorised account holder.

5. Prize Draws and Verification of Potential Winners: Winning is contingent upon complying with all terms and conditions of, and fulfilling all requirements contained in, these Official Rules. The Administrator, an independent judging organisation, will randomly select one (1) potential Prize Draw winner from all eligible entries received from the residents of that country during each Entry Period on the dates specified in Section 3 above. Each draw will take place at the Administrator’s offices at the address set forth above at around 17:00 GMT. Entries, whether winning or non-winning, will not be carried over into, and will not be included in any subsequent Entry Period drawings. The potential winners shall be notified either by e-mail, telephone, or registered post after the date of the Prize Draw. Except where prohibited, each potential winner may be required to sign and return winner documentation, as determined by the Sponsor at its sole discretion, to confirm eligibility, release the Sponsor and Administrator of liability or for any other reason deemed necessary by the Sponsor. If documentation is required, it must be received by the Sponsor or Administrator within the time frame indicated at the time of the initial win notification otherwise the potential winner forfeits the prize. If a potential winner cannot be contacted, fails to sign and return the required documentation (if applicable), does not answer the skill-testing question correctly (if applicable) or the prize is returned as undeliverable, the potential winner forfeits the prize. Potential winners must continue to comply with all terms and conditions of these Official Rules and winning is contingent upon fulfilling all requirements. In the event that a potential winner is disqualified for any reason, the Administrator will award the applicable prize to an alternative winner via a random draw from among all the remaining eligible entries from the applicable country. Only three (3) alternative draws will be held, after which the prize will remain un-awarded. Prizes will be fulfilled approximately ten (10) to twelve (12) weeks after winner verification. Any un-awarded prize(s) will not be combined with the prize(s) for, or otherwise carried over to, any subsequent Entry Period.

6. Prizes:

United States Prizes: FOUR (4) UNITED STATES PRIZES (one (1) per Entry Period): $12,000 USD. Total Value of United States Prizes: $48,000 USD.

Canada Prizes: FOUR (4) CANADA PRIZES (one (1) per Entry Period): $1,500 CND.  Total Value of Canada Prizes: $6,000 CND.

United Kingdom Prizes: FOUR (4) UNITED KINGDOM PRIZES (one (1) per Entry Period): £5,000. Total Value of United Kingdom Prizes: £20,000.

Netherlands Prizes: FOUR (4) NETHERLANDS PRIZES (one (1) per Entry Period): €2,000. Total Value of Netherlands Prizes: €8,000.

For All Prizes: Prizes will be awarded as cash or a cash equivalent. Winners may be required to set up a money transfer account, as directed by the Sponsor, in order to receive his/her prize. Prizes are non-transferable and no substitution will be made except as provided herein at the Sponsor’s sole discretion. The Sponsor reserves the right to substitute a prize for one of equal or greater value if the designated prize should become unavailable for any reason. Neither the Sponsor nor the Administrator shall be liable or responsible for any lost, stolen or misplaced prizes. Winners are responsible for all taxes and fees associated with prize receipt and/or use. The odds of winning a prize depend on the number of eligible entries received from individuals residing within each eligible country during each Entry Period. Limit: One (1) prize per person, per Entry Period.

7. Release: By entering the Prize Draw, each entrant agrees to release and hold harmless the Sponsor, Administrator and their respective subsidiaries, affiliates, suppliers, distributors, advertising/promotion agencies and prize suppliers, and each of their respective parent companies and each such company’s officers, directors, employees and agents (collectively, the 'Released Parties') from and against any claim or cause of action, including, but not limited to, personal injury, death or damage to or loss of property, arising out of participation in the Prize Draw or receipt or use or misuse of any prize. This Release does not apply to claims for personal injury or death caused by the negligence of the Released Parties.

8. Publicity: Except where prohibited, participation in the Prize Draw constitutes the Winner’s consent to the Sponsor and its agents’ use of the Winner’s name, likeness, photograph, voice, opinions and/or home town/city and county for promotional purposes in any media, worldwide, without further payment or consideration.

9. General Conditions: The Sponsor reserves the right to cancel, suspend and/or modify the Prize Draw, or any part of it, if any fraud, technical failures or any other factor beyond the Sponsor’s reasonable control impairs the integrity or proper functioning of the Prize Draw or, as determined by Sponsor at its sole discretion. The Sponsor reserves the right, at its sole discretion, to disqualify any individual it finds to be tampering with the entry process or the operation of the Prize Draw or to be acting in violation of these Official Rules or in an unsportsmanlike or disruptive manner. Any attempt by any person to deliberately undermine the legitimate operation of the Prize Draw may be a violation of criminal and civil law, and should such an attempt be made, the Sponsor reserves the right to seek damages from any such person to the fullest extent permitted by law. The Sponsor’s failure to enforce any term of these Official Rules shall not constitute a waiver of that provision. 

10. Limitations of Liability: The Released Parties are neither liable or responsible for: (1) any incorrect or inaccurate information, whether caused by entrants, printing errors or by any of the equipment or programming associated with or utilised in the Prize Draw; (2) technical failures of any kind, including, but not limited to malfunctions, interruptions or disconnections in phone lines or network hardware or software; (3) unauthorised human intervention in any part of the entry process or the Prize Draw; (4) technical or human error which may occur in the administration of the Prize Draw or the processing of entries; (5) late, lost, undeliverable, damaged or stolen post; or (6) any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from the entrant’s participation in the Prize Draw or receipt or use or misuse of any prize. If for any reason an entrant's entry is confirmed to have been erroneously deleted, lost or otherwise destroyed or corrupted, the entrant’s sole remedy is another entry in the Prize Draw, provided that, if it is not possible to award another entry due to the discontinuance of the Prize Draw or any part of it, for any reason, the Sponsor, at its sole discretion, may elect to hold a random draw from among all the eligible entries received up to the date of discontinuance for any or all of the prizes offered herein. No more than the stated number of prizes will be awarded. In the event that production, technical, seeding, programming or any other reasons cause more than the stated number of prizes as set forth in these Official Rules to be available and/or claimed, the Sponsor reserves the right to award only the stated number of prizes via a random draw among all legitimate, un-awarded, eligible prize claims.

11. Disputes: Except where prohibited, the entrant agrees that: (1) any and all disputes, claims and causes of action arising out of or connected with this Prize Draw or any prize awarded shall be resolved individually, without resort to any form of class action and exclusively by the United States District Court for the Eastern District of Michigan (Southern Division) or the appropriate Michigan State Court located in Oakland County, Michigan, U.S.A.; (2) any and all claims, judgements and awards shall be limited to actual out-of-pocket costs incurred, including the costs associated with entering this Prize Draw, but in no event lawyers’ fees; and (3) UNDER NO CIRCUMSTANCES WILL THE ENTRANT BE PERMITTED TO OBTAIN AWARDS FOR, AND THE ENTRANT HEREBY WAIVES ALL RIGHTS TO CLAIM, INDIRECT, PUNITIVE, INCIDENTAL AND CONSEQUENTIAL DAMAGES AND ANY OTHER DAMAGES, OTHER THAN FOR ACTUAL OUT-OF-POCKET EXPENSES AND ANY AND ALL RIGHTS TO HAVE DAMAGES MULTIPLIED OR OTHERWISE INCREASED. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of the entrant and Sponsor in connection with the Prize Draw, shall be governed by, and construed in accordance with, the laws of the State of Michigan, U.S.A. without giving effect to any choice of law or conflict of law rules (whether of the State of Michigan, U.S.A. or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the State of Michigan, U.S.A.

Entrant's Personal Information: All personal information submitted to the Sponsor and/or the Administrator will be treated in accordance with ePrize’s privacy policy at http://www.eprize.com/privacy-policy and the Sponsor’s privacy policy (applicable to the entrant's country and the survey taken) as outlined in the chart below:

Entrants have the right to access, withdraw, and correct their personal information. The personal information submitted, received or collected in connection with the Prize Draw shall be stored on a server within the United States of America. Entry into the Prize Draw constitutes your acknowledgement of, and explicit consent to, the export of your personal information to the United States of America and its use there for the purposes of administering this Prize Draw and associated processing (e.g. but not limited to back up, archiving). This server is controlled by the Sponsor and/or the Administrator who have notified the US Department of Commerce that they adhere by the rules of the Safe Harbour Framework available under http://www.export.gov/safeharbor/eg_main_018236.asp.

14. General: Nothing in these terms and conditions shall be deemed to exclude or restrict any of the winner's or entrant's statutory rights as a consumer.
